Which data and why

Patients

To treat you responsibly we record: name, date of birth, contact details, your medical history, medication and allergies, the treatment agreement and treatment notes, and where needed photographs of the treated area. Legal basis: performance of the treatment agreement and our statutory duties as a healthcare provider (WGBO, Wkkgz).

Invoicing

Name, address and payment details for invoicing and our tax retention obligation. Legal basis: legal obligation.

Contact and appointments

If you contact us by e-mail, WhatsApp or the online booking system, we use your name and contact details solely to answer your question or to schedule and confirm the appointment.

Retention periods

• Medical record: 20 years after the last change, in accordance with the WGBO.
• Financial records: 7 years, in accordance with the tax retention obligation.
• Contact requests without treatment: until your question is handled, at most one year.

Who we share data with

Only with parties needed to run the care and the practice, under a data processing agreement:

• Clinicminds — secure electronic patient record and online calendar.
• Our e-mail provider — for correspondence.
• Our bookkeeper — for financial administration.
• Cloudflare — hosting of this website.

We do not sell data and never share your medical data with third parties without your explicit consent, unless the law requires it. Images are used for purposes other than your record only with your written consent.

Cookies and this website

This website sets no cookies and uses no analytics or marketing trackers, which is why you see no cookie banner. When you click "Book", the Clinicminds booking system opens; the privacy statement of Clinicminds applies there. Fonts are loaded via Google Fonts.

Your rights

You have the right to access, correct and (within legal limits) erase your data, to data portability and to object to processing. Send your request to info@edwardsaesthetics.com; to protect your record we may ask you to prove your identity. You receive an answer within one month. If you are not satisfied with how we handle your data, you can lodge a complaint with the Dutch Data Protection Authority, the Autoriteit Persoonsgegevens.

Security

Records are stored in a secure system used in healthcare, with access control. Only Dr. Edwards and, where needed for care, persons authorised by her have access to your record.

In case of any difference between this translation and the Dutch text, the Dutch text prevails.
